We are very honored by your choice to come with us on our new adventure! If you previously scheduled an appointment at our current practice we will do our best to keep that spot reserved for you. Although we have been working together, we will need you to complete a few steps as if you were a new client. Please start this as soon as possible so we can get your account created and insurance approved. We will not be able to see you until this is done.
